Sample Online Restaurant Reservation Form

Please fill out all the following information to make dinner reservations with us. WWW reservations must be made at least 24 hours in advance, and will be confirmed either by phone or email by our staff. For reservations less than 24 hours in advance, please call us.
Filling out this form also puts you in our mailing list. This will keep you informed of any special events that we may offer in the future. If you prefer to not be on our mailing list, please answer accordingly in the question at the at the bottom of this page.
Beginning June 1st we will be open 7 days a week. Other days, We will be hosting private parties and special events. Persons or companies interested in having a private dinner party at our restaurant should send email to dining@myrestaurant.com or call at 824-695-3487.
